Project Overview:

Provide a brief history and overview of your organization, including your mission statement and goals.
Describe the specific purposes for which any infrastructure grant funds awarded from Helpers Community, Inc. will be used. Note: This year is for infrastructure funding only (i.e., roof/flooring repairs/replacement, plumbing/electrical system upgrades, fire safety equipment, conference tables and office furniture, equipment, etc.).

List the specific SMART (specific, measurable, attainable, relevant and time-based) objectives of the project to be funded. Be as specific as possible in describing the tangible and measurable changes or results realized by your clients due to this funded project. (i.e., improved service delivery, increased number of clients served, enhanced safety and compliance, organizational sustainability, etc.). attendance, completion, year-to-year retention rates, milestones).
Describe your organization’s implementation plan and timeline for the program. (Include # of Staff & Volunteers needed). Describe how your organization will achieve the objectives of your project and the specific actions it will take to achieve results.
Describe any potential implementation challenges and what the strategy will be to meet those challenges. (i.e., permits, unforeseen costs). Do you anticipate a lack of funds from Federal, State and/or Regional Center grants or contracts? If so, what potential impact could this have on your budget process and operations. How do you anticipate managing or addressing this challenge? Given obstacles, how realistic is implementation during this calendar year?
